Will I Be Covered For My Co-op Work Term?
Yes.
Students on Fall Co-op work term will be billed for 4 months coverage (September 1st - December 31st, 2010) in the fall and the remaining 8 months coverage (January 1st - August 31st, 2011) when you return to school for the winter term.
- Winter co-op work term students that were billed the full-year fee in the fall will be automatically covered.
- Spring co-op work term students that were billed in either the fall or in winter will automatically covered.
Why a Student Health & Dental Plan?
A growing number of important health-care services are not covered by provincial health care. For many students, paying for these services can lead to financial hardship, while other cannot afford them at all. To help students pay for the health services they need, a student health and dental plan has been put in place by your student association. To use the health benefits, you must be covered by provincial health care coverage (such as OHIP) or the equivalent. The Health & Dental Plan is uniquely designed for, providing a comprehensive package of health and dental benefits to fill the gaps left by provincial health care and a parent or spouse's plan.
Who’s Covered?
The Student Union Health & Dental Plan automatically covers all full-time students and day part-time students who are enrolled to Sheridan college. This does not include international students.
What Is Covered & How Much Does It Cost?
The plan included benefits for prescription drugs, vaccinations, physiotherapists, chiropractor, massage therapists, vision care, dental benefits, travel health coverage, and more. For a complete list of benefits and details please visit the Student Care website.=
- The annual plan fee is $180.00. The policy year is from September 1st, 2010 - August 31st, 2011.
- The fee for new winter students is $120.00 for 8-months (January 1st - August 31st 2011) of coverage.
- The fee for new spring students is $60.00 for 4-months (May 1st - August 31st 2011) of coverage.
These fees are automatically billed to your student account (instructions on how to opt out are below)
How Do Students Claim Benefits?
A pay-direct card is available at ihaveaplan.ca or in the Plan pamphlets, which can be used for Prescription drug claims. By presenting the Pay-Direct Card with valid student identification, students can fill their prescriptions at most Canadian pharmacies and receive 80% off the price cost of prescriptions at the time of purchase. The pharmacist will be able to process the claim immediately, so they won't have to pat the full amount up from and wait to be reimbursed.
For vision and other health claims (including prescription drugs if a student does not use the Pay-Direct Card), students will be responsible for payment when the service is rendered. Students can then complete and sign an extended Health Claim Form, attach any necessary receipts and/or documents, and mail them to the insurance company (Sun Life) or drop them off the Student Union Office.
For dental claims, students must complete a claim form with their dentist. Some dental offices can do an electronic submission; in this case, students will only be required to pay for the uninsured portion of the service. Other dental offices will have you pay the full fee for all services and then file the claim for reimbursement. Claim forms are available at ihaveaplan.ca or at your campus' Student Union office. Photocopies of blank claim forms may also be used.
Can I Enroll My Family?
Yes.
The Health & Dental Plan gives you the option to enroll your family (spouse and/or dependents) by completing an enrolment process and by paying an additional fee, over and above the fee as a member of the Student Union. Common law and same-sex couples are eligible. All enrollments must be completed within the change-of-coverage period and are for a full-year of coverage (for all enrollments). New winter term students may enrol their spouse/dependents during May change-of-coverage period.
